When is my survey due?

View the Schedule of Surveys & Results page, which provides the specific due date for each questionnaire.

How do I login to access the survey or results?

Your login ID and password information will be sent via email to the primary contact for your organization. If you cannot locate this email, call (800) 300-9570 for your login information.

How much information is required to complete the survey in full?

To keep your discounted participant rate, you must fill out the Company Profile, Benefits and Jobs Questionnaires. In the Jobs Questionnaire, match each description to the corresponding position in your organization and report the data. We encourage each participant to make every effort to provide as much data as possible in order to maintain the quality of the results.

Where can I find the list of job titles?

Have any of the job titles or numbers been updated for next year’s survey?

We regularly make updates to the job titles included in each survey. Updates for next year’s survey will be posted here soon.

My company has several different benefit plans for different employees. What should I report?

Once you have completed one benefit plan, you may use the “Copy Plan” button at the top of the page. This will open up a second copy of the original plan, and you may edit as needed. You may copy and edit as many benefit plans as you need, or start a new blank plan.

How do I report locations in more than one region?

You will need to complete an entire survey (all three sections) for each region. To speed completion for more than one region, the program has the ability to copy the completed Company and Benefits sections from one region into a new one. Once copied, make changes that apply to the new region. You will be able to view, update and/or enter all of your jobs, regardless of zip code, under any region.

How can I access my data from last year?

Participants who completed the survey last year will have their responses pre-populated in the current year’s online survey.  This will save you time when completing the survey.  Review your pre-populated responses carefully and make changes where appropriate. If you use the spreadsheet template for the Jobs Questionnaire, you will see last year’s data populated in the spreadsheet once you select the “Download Spreadsheet” link.

After I have completed the survey, how do I submit my responses?

If you have entered your job data using the spreadsheet template, you will need to validate and upload your spreadsheet.  Click on the “Validate & Upload My Spreadsheet” link under the Job Questionnaire section of the Survey Menu.  Follow the instructions on this page.  If the validation process returns errors, you will need to correct the errors and re-upload your spreadsheet.

After completing the entire survey, choose the “Survey” tab to return to the Survey Menu. To submit your survey, select the “I’m done with the survey” button at the bottom of the screen.  A “Survey Audit” report will appear in a new window. Review the results of the audit and check any pieces of flagged data. Once you are satisfied with your survey responses, you can select the “Submit” option.  If you decide you are not ready to submit your questionnaire, you can select “Cancel” and be directed back into the survey.

Note: You will need to submit each survey separately. For example, if you are completing multiple regions or industries, each region/industry must be submitted on its own.
